Qingdao blasts blamed on human error, paper says

A “very serious dereliction of duty” caused Friday’s deadly oil pipeline blasts in Shandong province, China Daily reported Monday, citing a senior work safety official. Yang Dongliang, director of the State Administration of Work Safety and head of the team investigating the blast, said the incident exposed three severe problems: poor layout of pipelines and drainage systems, negligence in pipeline monitoring, and poor handling of oil leaks before the explosions, the report said.
